1. No purchase necessary or requested.
2. To enter, answer all survey questions and submit your completed entry back by clicking the ‘Submit answers now’ button. All questions must be answered for entry. No facsimile or regular mail entries permitted. Only one entry per person per month.
3. The contest is open to anyone eighteen or over except employees and their immediate families of Cherokee Uniforms, its affiliates, subsidiaries, advertising and promotion agencies. All submissions become the property of Cherokee Uniforms. All federal, state and local laws and regulations apply. Void where prohibited or restricted by law. All federal, state and local taxes are the responsibility of the winners.
4. Sweepstakes ends at the end of the month. The sweepstakes drawing will be held early next month. Three winners will be selected in a random drawing from among all completed entries received under the supervision of Cherokee Uniforms, whose decision will be final. The odds of winning depend on the number of entries received. Winners will be notified by e-mail. Any prize notification or prize returned to Cherokee Uniforms as undeliverable will result in awarding that prize to an alternate winner. Where permitted, acceptance of a prize constitutes consent to use of winner’s name in any manner by Cherokee Uniforms and its advertising and promotion agencies without further compensation.
5. The following prizes will be awarded each month:
3 winners will receive 1 scrub top each.
6. Winners may be required to submit a signed Affidavit of Eligibility/Release of Liability/Prize Acceptance Form certifying that they are eligible to enter the contest. If required, the affidavit must be signed and returned within 15 days of attempted notification. Failure to return a properly completed affidavit within this period may result in selection of an alternate winner. Winner may make no substitution or transfer of prize. Cherokee Uniforms reserves the right to substitute a prize of equal or greater value.
7. Neither Cherokee Uniforms nor its affiliates, subsidiaries, division, or related companies are responsible or liable for: (1) any damages, taxes, expenses, losses or injuries resulting from winners acceptance or use of any prize; (b) lost, misdirected or late entries, computer malfunctions, online failures, technical malfunctions or disturbances or other circumstances affecting receipt of entries or operation of the contest; (c) any warranty, representation, or guarantee either express or implied on any prize awarded (except as may be expressly warranted by Cherokee Uniforms) including, but not limited to its quality, condition or fitness.
